Application for Credit by Examination (PDF)<\/a><\/strong><\/td>This allows you the opportunity to receive college credits for what you already know! Students can demonstrate their skills and knowledge received through prior learning from non-accredited institutions and\/or work experience.<\/td><\/tr>
Application for Graduation (PDF)<\/a><\/strong><\/td>If you are close to completing your degree, we suggest you meet with the academic advising office to make sure you are ready for graduation. If so, please complete the graduation application online thru Ocean Connect\/Student Planning. If you are unable to log in or submit an application online, then you may use this form. You will be evaluated, and a letter will be mailed indicating if you have been approved for graduation.\u201d<\/td><\/tr>
*Commit to Complete<\/a> <\/strong><\/td>You may be eligible for a free 3-credit class. View qualifications here<\/a>. You must complete and submit this form once you are registered for the summer class to be evaluated for the free class.<\/td><\/tr>
Change of Curriculum<\/strong> (PDF)<\/a><\/td>If you would like to update your program of study, please meet with the academic advising office. Once an academic advisor signs off on this form, you may submit it for your curriculum to be updated.<\/td><\/tr>
Chargeback Procedure (PDF)<\/a><\/strong><\/td>chargeback assistance will be available for Ocean County residents who enroll in a course or program, at another county college in New Jersey, that is not offered at Ocean County College.<\/td><\/tr>
*Driver\u2019s License Verification Form<\/a> <\/strong><\/td>Use this form to upload a copy of your driver\u2019s license.<\/td><\/tr>
Drop\/Add (PDF)<\/a><\/strong><\/td>Most students have access to add\/drop courses on Student Planning. However, if you receive a block notification, or need an advisor\u2019s or instructor\u2019s signature, fill out this form.<\/td><\/tr>
*Early College Grant Application<\/a><\/strong><\/td>Application for High School students taking College Courses at OCC or at their High School.<\/td><\/tr>
Early College Parental Consent Form (PDF)<\/a><\/strong> <\/td>Required for high school students.<\/td><\/tr>
Enrollment Verification Request<\/a><\/strong><\/td>After the census date of the semester, students can request an enrollment verification. You can request an enrollment verification free of charge at any time by completing this form and submitting it with a copy of your government-issued photo ID.<\/td><\/tr>
Immunization Record<\/a> <\/strong><\/td>In accordance with New Jersey law, all new on-campus students carrying 12 or more credits are required to submit a record of immunization against measles, mumps, rubella, and hepatitis B or indicate exemption due to medical or religious beliefs.<\/td><\/tr>
Incomplete Grade Request<\/a><\/td>In order to be eligible for a grade of \u201cI\u201d (Incomplete), a student shall have a reasonable chance at passing the course at the time of the request, as deemed by the faculty member. You must communicate with faculty before completing and submitting this form.<\/strong> Review form for details.<\/td><\/tr>
Nursing Letter of Intent<\/a><\/strong> <\/td>Complete this form if you are interested in the Traditional Nursing Program. You must submit this form for every semester<\/strong> that you wish to be considered for the traditional nursing program.<\/td><\/tr>
Mobilized Military Students Request<\/a><\/td>Pursuant to\u00a0N.J.S.A. 18A:62-4.2,<\/a>\u00a0a student who is unable to complete a course\/s due to deployment, mobilization, reassignment, or other military obligations as a service member should complete this form, and shall be entitled to the options set forth in this section with respect to the student\u2019s grade for the course. Reference Policy #5335.<\/a><\/td><\/tr>
Registration Form (PDF)<\/a><\/strong><\/td>Eligible students are welcome to register via their Ocean Connect account. However, if you are in need of assistance and must register manually, you may use this form.<\/td><\/tr>
*Student Information Change<\/a><\/strong><\/td>If you need to correct your name or address, please complete this form along with a copy of your government-issued photo ID with the updated name and\/or address. We will also accept the confirmation from the Motor Vehicle Commission. Leases, utility statements, and bills are not accepted as address documentation.<\/td><\/tr>
Student Social Security Number Form<\/a><\/strong><\/td>Securely submit your Social Security number for Financial Aid purposes.<\/td><\/tr>
Transcript Request<\/a><\/strong><\/td>We no longer have paper transcript request forms. All students must order a transcript via our ordering website<\/a> through the National Student Clearinghouse.<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n
